She stands with her hands lowered and open in the attitude of the Miraculous Medal, and the oval sits slightly proud of the blossoms, so she keeps her place at the center of the ornament.
The image is the one St. Catherine Labouré was given in the Rue du Bac in Paris in 1830, with rays of grace falling from Our Lady's open hands to all who ask for them.
